Patient came to hospital with an obstructing colon cancer. This was opened with a colonic stent. The patient was successfully resected a week later. Stent performed by Dr. David Schaffzin at the Center for Colon and Rectal Health, St. Mary Medical Center, Langhorne, PA

A 50 years old male presented with abdominal distension and vague abdominal pain. Was found to have a huge GIST tumor at about 50 cm from the DJ junction, firmly attached to the bowel wall, but no obstruction. The tumor was resected with segmental resection of the adjacent small bowel and primary anastomosis. The tumor was ~ 30x30 cm and 6 kg. The patient had a smooth recovery.

This 38 year old male presented in surgery emergency with complain of pain abdomen and not able to pass stool and flatus for one day. On examination his abdomen was distended. There was a huge ventral hernia in supraumbilical region which was tense , tender and irreducible. bowel sounds were exaggerated. X-ray showing multiple air fluid level. Emergency surgery planned. Transverse incision given over hernia swelling. Hernia sac separated . Gangrenous bowel loops found inside sac. Sac opened , around one and half feet small bowel resected and end to end anastomosis done . Hernia defect closed primarily with 1-0 prolene suture without mesh placement. patient recovered well and discharged on 4th day. Discussion-- Women are affected more than men , ratio 5:1 Patient usually in age group of 35- 50 years. Causes- 1- Increasing obesity 2-Flabby abdominal muscle 3-Repeated pregnancy Usually present with dragging pain with gastrointestinal symptoms. Usually there is adhesion between sac and omentum leading to irreducible hernia. Loops of bowel may remain as content and can lead to strangulated hernia with bowel ischemia. Management of paraumbilical hernia. Surgery is the only choice and should not be delayed. Small hernia defect- Umbilical hernioplasy (Mayo repair) is good enough. Diagnostic laparoscopy is a procedure that allows a health care provider to look directly at the contents of a patient's abdomen or pelvis, including the fallopian tubes, ovaries, uterus, small bowel, large bowel, appendix, liver, and gallbladder. Evan Levy was a busy, active senior at Cornell University. He had always dreamed of becoming a doctor and was about to begin applying to medical schools. After mysteriously developing severe, prolonged gastrointestinal symptoms, he began a course of medical therapy prescribed by physicians in his home state of Florida. His condition became so debilitating that he had to put his applications on hold and almost didn't make it to graduation. When he wasn't getting better, Evan and his mom flew up to NYP/Weill Cornell, where they saw gastroenterologist Dr. Brian Bosworth. It was immediately apparent that Evan had severe ulcerative colitis and needed surgery right away. Within days, Evan was in the OR with colorectal surgeon Dr. Toyooki Sonoda. He began a series of multiple surgeries and procedures that spanned over the next 10 months to remove Evan's entire large bowel and remodel his small intestine to restore bowel function.
